SUPPORT COORDINATION LEVEL 3
(Specialist Support Coordination)

Barkuma provides Support Coordination Level 3 (Specialist Support Coordination) for participants who require additional support in managing complex needs or situations.  SSC is high-level support coordination service that is designed to assist participants with complex support needs to navigate and access the National Disability Insurance Scheme (NDIS) and broader systems of support.

Specialist Support Coordination may be suitable for individuals with multiple disabilities, challenging behaviours, or complex health needs, as well as those who require assistance in coordinating supports from multiple service providers.

The role of Specialist Support Coordinators is to work closely with the participant and their support network, including family members, carers, and service providers, to develop a comprehensive support plan that meets the participant’s individual needs and goals. This may involve coordinating a range of supports and services, including health care, housing, employment, education, and social activities. Barkuma’s Specialist Support Coordinators are highly skilled and experienced professionals who have a deep understanding of the NDIS and the broader disability sector. They work closely with the participant to identify their needs, strengths, and goals, and to develop a tailored plan that maximises independence, social inclusion, and quality of life.
